Providing your login details allows us to securely retrieve the required files on your behalf and save you time. However, if you prefer, you're welcome to attach the files directly in this form.
Note: All information is securely transmitted and permanently deleted from our system after your case files are received.

It only takes a few quick steps to create one 👇
- 🔗 Go to myappeal.cuiab.ca.gov
- 📱 On the sign-in page, click Register
- ✅ Accept the Terms and Conditions
- 👤 Choose your role: Claimant
- 📝 Fill in your case number, name, email, and phone
- 📧 Check your email for a confirmation link
- 🔓 Click the link to verify and finish setting your password
- 🎉 That’s it! You can now log in and access your appeal documents
